Every Evidence Summary must have an accompanying Technical Development Report (TDR). Download and complete this TDR template for each Evidence Summary developed or updated.
Prior to you first payment being processed, JBI must receive a correctly completed Statement by a Supplier Form. Downloadable from the Australian Tax Office Website.
Payment will occur via electronic funds transfer (EFT). Prior to the first payment, Scientific Writers will need to complete a ‘Banking Authority’ form and return this to JBI.

NOTE:
- In section 1, no 5 please add 'JBI EBP database'
- In section 2, please tick 'YES' and complete as per below. In the comments box, add: In the interest of full disclosure a fee for service was provided by JBI for updating Evidence summaries on the JBI EBP database. If you need to include any potential conflicts of interest, click add
- Complete all other questions as appropriate and generate disclosure statement.
