NEW JBIC ENTITY CHECKLIST

JBIC INVOICE TEMPLATE

Entities are required to complete the JBIC Entity Administration form when they are first accepted into the JBIC. This form is also used to update the Global Engagement Office with any changes to your Entity (such as staff updates). Complete the form and return to the Global Engagement Office jbic@adelaide.edu.au, we will then inform you once these changes have been made.

NOTE:

·         If, for example, you are adding a new staff member you are only required to add the details of that one person, it is not expected that you list your whole team each time.

·         On average we request each Entity to re-complete the form in its entirety once a year to ensure that our records are up-to-date.

To assist you with ensuring you have completed all the necessary documents and provided us with everything that we need to get you started with the JBI Collaboration please ensure you go through the checklist thoroughly.

The JBI Invoice Template should be completed by JBI Centres of Excellence for travel reimbursement to attend the face to face JBIC AGM and/or for eligible JBIC Core Activity funding, as communicated by the Global Engagement Office. Centres of Excellence must only invoice for the amount in $AUD communicated by the Global Engagement Office.

To register for Clinical Partnership Engagement (refer to the JBIC Handbook - JBIC Matrix Activity), please download the form, complete, and return to the Global Engagement Office via email jbic@adelaide.edu.au

Mentors must come from a Centre of Excellence and work with their Mentee group/s to complete the JBIC Mentorship Registration form, for further information refer to the JBIC Handbook - JBIC Activity Matrix. Please download the form, complete, and return to the Global Engagement Office via email jbic@adelaide.edu.au

Where JBIC Entities work collaboratively on an activity and wish to share the allocation of points for that activity, Entities must complete the JBIC Activity Matrix Points Allocation form. Please download the form, complete, and return to the Global Engagement Office via email jbic@adelaide.edu.au

AFFILIATED GROUP APPLICATION FORM

HOST A JBI COLLOQUIUM APPLICATION FORM

Email the Global Engagement Office at jbic@adelaide.edu.au for an application form. Prior to requesting the form please ensure that you have familiarised yourself with the JBI Collaboration Handbook as this outlines the terms and conditions of becoming an Affiliated Group. If you have any questions feel free to email them to us, we are happy to help.

Following the call for applications from the Global Engagement Office to Host a JBI Colloquium, any JBI Centre of Excellence that is interested in hosting must complete this proposal and submit to the Global Engagement Office by 30 July, two years prior to the proposed Colloquium date.

JBIC REGIONAL ANNUAL REPORT TEMPLATE

JBIC REGIONAL OPERATIONAL PLAN

The JBIC Regional Annual Report must be completed by JBIC Regional Chair and submitted by 31 January each year. Remuneration for Regional Chairs is dependent upon timely submission of this Report.

The JBIC Regional Operational Plan must be discussed in the regional meetings and submitted by the corresponding Regional Chair by 31 January each year. Remuneration for Regional Chairs is dependent upon timely submission of this Plan.

EVIDENCE SUMMARY REQUEST FORM

A request form for JBI to undertake a JBI Evidence Summary on a desired topic. Must be used in conjunction with the Evidence Summary and Audit Criteria Request Policy
